The nationally-recognized Advocate Physician Partners Clinical Integration Program combines evidence-based medicine with an innovative pay-for-performance program. This partnership fosters collaboration among payers and patients, as well as physicians and hospitals, creating the framework of our ACO. The Clinical Integration Program is built on carefully researched metrics and established performance targets.
Medical Management
Practice management has become a complex and time-consuming task. As a physician-led organization, we support your desire to focus on practicing medicine by assisting the physician practice with day-to-day functions such as credentialing, eligibility and referral, utilization and case management services.
Contracting
Advocate Physician partners contracts with all major health plans in the Chicago metropolitan area and select plans in Central Illinois, providing broad access to care for patients and competitive contracts for its physician members. As a member of APP, you’ll enjoy a number of benefits related to our contract portfolio, including: competitive reimbursement rates, additional earning opportunities based on Clinical Integration Program performance, and non-exclusive contracts that give you the freedom to participate in other contracts.
Technical Support
Health care technology represents a tremendous investment for any practice. Members have access to a suite of tools designed to streamline office processes and improve patient safety, including an ERMA (electronic referralmanagement application), SynAPPs (electronic medical record program) and the Cerner registry system.
